My Commitment to Your Privacy
I acknowledge that as my Client you have a reasonable expectation of privacy and security regarding your personal information and I understand your need to protect your private information.
I therefore commit that any information you provide to me in the course of your relationship with ACL Chiropractic will be treated with the utmost privacy and security at all times.
How ACL Chiropractic Collects, Uses and Retains Client Information
I collect, use and retain patient information for a variety of purposes.
I collect personal information about your health in order to fully understand your treatment needs and to be able to provide you with chiropractic care. Without the benefit of this information I could not provide chiropractic care safely and effectively. You can, of course, refuse to provide this information, but in that instance I would not be able to provide treatment.
Some of the information is collected and retained in order to satisfy legal or regulatory reporting requirements. I have a legal obligation to retain your records for 8 years after your most recent appointment (or until you are aged 25, if this is longer) but after this period you can ask me to delete your records if you wish. Otherwise, I will retain your records indefinitely in order that I can provide you with the best possible care should you wish to see me at some future date.
Some of the information I collect about you is non-public personal information for example, your name and address. I may use this type of information to contact you in order to arrange/confirm your appointments with me or to update you on matters related to your medical care.
Maintaining Accurate Information
I have clear defined procedures to ensure that the information that I hold about you is accurate, current and complete, in accordance with commercial standards.
I commit to respond to your requests to correct inaccurate information in a timely manner.
My Security Procedures for Protection of Your Information
I maintain security standards and procedures that help to prevent unauthorised access to Client information that conforms to common industry practices.
My Commitment on Disclosure of Your Information to Third Parties
I commit that I will not reveal non-public personal information about you to third parties for their independent use unless
(1) you have expressly requested or authorised it and
(2) the information is necessary in relation to any ongoing treatment
(3) such disclosure is otherwise lawfully required or permitted.
I do not share non-public current of former Client information with third party marketeers.
I commit not to disclose any non-public current or former Client information to anyone with the exception of the above provisions in this paragraph.
It is a strict ACL Chiropractic policy to not knowingly solicit data from children.
You have the right to access any and all of the information I hold about you. You can do this by contacting Anne Loader who is the ‘data controller’.
